CareCredit Instructions

You are receiving this information to help you understand Discover Vision’s financing options through CareCredit.  If you should decide to take advantage of the financing options that are available to you, the following guidelines must be adhered to prior to your surgery day.  **PLEASE NOTE: EVEN IF YOU ARE ALREADY A CARECREDIT CARD HOLDER, YOU MUST NOTIFY US AT LEAST 1 WEEK PRIOR TO YOUR SURGERY DATE TO ENSURE THAT PAPERWORK IS COMPLETE.

Step 1: Applying for CareCredit Financing

  • During your evaluation or pre-operative exam, you will be given CareCredit information.  You can apply:
    • In the office:  This is a simple process which takes approximately 15-30 min. for paperwork and approval status.  We will need 2 forms of I.D, as well as personal information, such as date of birth, social security number, home phone, physical address, monthly net income, etc.  If you wish to have a co-signer, they must be present during this time to sign the necessary consent forms.
    • Online by visiting www.discovervision.com:  Click on the Finance & Billing tab, then Finance.  Scroll toward the bottom of the page and click “Apply Now Online.”  Fill out the necessary information, click the “Accept & Submit” button, and you will receive an immediate answer.
    • Over the phone: Call 800.365.8295 and complete an application with a CareCredit representative.
Step 2: Acceptance
  • In the office:  If you apply in the office with one of our refractive educators and are approved, skip to step 3.
  • From home:  Once you have received your approval notification page or acceptance letter, please call our office at 816.478.1230 or call one of the refractive educators listed below.  We will need the following items:
    • Account number
    • Credit limit
    • If possible, the reference number for your application
Step 3: Choosing a Payment Plan
  • Choose the CareCredit payment plan you would like to use.  Discover Vision offers the following options:

Step 4: Make Arrangements to Sign Paperwork
  • Once we are able to verify your account information, we will make arrangements for you to sign your paperwork prior to the day of your surgery, unless otherwise specified.  This must be done even if you are already a CareCredit card holder.
Step 5: Day of Surgery
  • If the above steps are followed, your check-in process at our Surgery Center should run smoothly.  However, not following these steps will lead to longer wait times the day of your surgery.
