We understand how difficult it is to verify insurance coverage while also determining your level of personal responsibility. Medical and routine eye care service involves verification of your health insurance and vision insurance if applicable. We would be happy to verify your insurance coverage for you. To do so, be sure to provide our staff with the following things prior to your appointment:

1. Name, date of birth and ID# on the card
2. Name of your insurance
3. Group name if applicable
4. Customer Service phone number usually located on the back of the card

Though we are willing to verify your insurance coverage for you, we do so as a courtesy.  Ultimately, you are responsible for any remaining balance that your insurance denies or deems as a non-covered service.  To ensure that you receive the proper coverage, please call your insurance company or refer to your insurance handbook.

NOTICE TO ALL PATIENTS:

The U.S. Department of Health and Human Services' Office of Inspector General issued an alert in 1994 that addresses the waiving of patient copays and deductibles. This alert indicates that waiving such payments is a violation of the False Claim Act and anti-kickback statutes. By law, no medical practice, including Discover Vision Centers, can waive or write-off copays and deductible fees. These fees have been decided and agreed upon by you, your insurance company and its contractual medical providers. Waiving fees at the time of service would be in violation of the contract our medical practice has made, as well as the agreement you have made with your insurance company. Because Discover Vision Centers and its providers can suffer civil and criminal legal repercussions, Discover Vision Centers cannot provide services unless the steps required by your insurance company are followed explicitly. This means that no copays, deductibles or other fees will be waived at the time of service. We appreciate your cooperation and understanding in this matter.
